Undertaking
- I declare that all data given in this form is complete and true. I am aware that any false statements can lead to my application being rejected. I understand and agree that my personal data may be collected, used and disclosed by AWWA to facilitate my volunteer service at AWWA. The personal data includes all personal information provided in this form, and/or any documents provided by me or from other sources. I understand that AWWA will collect, use or disclose personal data only for purposes that a reasonable person would consider appropriate in the circumstances.
- In applying to volunteer with AWWA, I understand that AWWA reserves the right to accept or decline my volunteer services, depending on the match between my skills or expertise and the current needs of AWWA. If accepted to do volunteering service, I will abide by the terms in AWWA’s Volunteer Handbook, a copy of which will be extended to me, and will seek to fulfil my commitment and responsibilities to the best of my abilities.
- I shall not hold AWWA, its directors, officers, employees, servants, agents and contractors responsible for any damage to or loss of property, any injury or illness which may be sustained by me as a result of any accident or mishap which may occur during my voluntary work and its related activities with AWWA. I also undertake to indemnify AWWA, its directors, officers, employees, servants, agents and contractors against any and all losses, damages, actions, proceedings, costs, claims, fines, demands by any party in connection with the above-mentioned activities.
- I agree and understand that all information (including any personal data) relating to or concerning AWWA and/or its clients (“AWWA information”) obtained or disclosed to me during the performance of or in connection with my volunteer services with AWWA is and must remain confidential. This includes all documents, pictures, videos, and all information about AWWA’s clients, their families, employees and other associate organisations, as well as any other information otherwise marked or known to be confidential.
- I will not collect, use or disclose any AWWA Information without the permission of AWWA. I shall not take/record or use photographs, video and audio recordings of AWWA and/or its clients during the course of my volunteer service without the prior written permission of AWWA.
- Where the AWWA Information relates to personal data, I agree and confirm that I will comply with the provisions of the Personal Data Protection Act 2012 in respect of the collection, use, disclosure and/or processing of such personal data.
- I further understand that any breach of the duty to maintain confidentially shall be grounds for immediate termination of my services as a volunteer and or possible liability in any legal action arising from such breach.
- I fully agree to the collection, use and disclosure of personal information by AWWA which I have provided including disclosure to other agencies or individuals for the purposes of (i) Volunteering related Opportunities, Updates and Activities, (ii) AWWA Publicity and Outreach, (iii) AWWA’s updates which may be of interest to me, (iv) Reports and Evaluation, (v) Screening by any government or regulatory authority.
- In connection with the volunteer services to be rendered to AWWA’s client(s), I agree and consent to the collection, use and disclosure of my personal data including my contact number to AWWA’s client(s) if necessary for the purpose of the relevant programme or service. I understand that while AWWA will explain to its client(s) to keep my personal data confidential, AWWA has no control over the client(s) and his/her ability to keep my personal data confidential. Accordingly, I will not hold AWWA responsible for any personal data breaches by the client(s). In addition I agree that I will not hold the client(s) responsible for any such personal data breaches.
- I understand that the AWWA Data Protection Policy and option to withdraw my consent given as set out in this undertaking is available on AWWA corporate website.
- AWWA may take photographs and/or record video/audio for services/activities which I am volunteering in. I understand and agree that I may appear in these photographs/video/audio and such materials may be used by AWWA for purposes including but not limited to publicity, annual reports, education and training and fundraising. I understand that volunteers will not be named in the photographs/video/audio used for such purposes and AWWA will take all reasonable measures to ensure that they are not put in negative light.
