Seniors

AWWA Senior Care Centre (SCC) supports adults with disabilities and seniors who have been discharged from hospitals or referred from the community for rehabilitation. Day care services are also provided for individuals with multiple co-morbidities, including dementia, and are delivered using a person-centred approach with care plans tailored to individual goals, and preferences.

It aims to maximise the potential of seniors through structured rehabilitation delivered by a multidisciplinary team, engage seniors in social and recreational activities during the day, and support caregivers through training, and referral services.
